You will perform assessments, develop and adjust plans of care, educate patients and families, and document care in the EMR. The role offers competitive per‑visit pay starting at $43 with mileage reimbursement, flexible

Qualifikationen

• RN license in SC or Compact required.
• Minimum one year of nursing experience.
• Graduate of an accredited RN program.
• Home Health, Med-Surg, CCU or Case Management experience preferred.
• Familiarity with OASIS documentation preferred.

Aufgaben

• Provide skilled, patient-centered care in the home with the team to support recovery.
• Perform comprehensive patient assessments and accurate OASIS documentation.
• Develop, implement, and adjust individualized plans of care.
• Monitor patient condition and communicate changes to physicians promptly.
• Educate patients and caregivers on medications, treatments, and home safety.
• Coordinate care with therapists, aides and other providers for seamless care.
• Prepare patients and families for discharge and document care in the EMR.
