Lorian Health in San Jose NW is seeking a Home Health RN for a weekend-focused, full-time role. You will deliver skilled nursing care per the patient’s Plan of Care, assess and monitor conditions, administer treatments, and educate families to support independent living at home.

Qualifications

• RN licensed in the state of practice.
• BSN preferred.
• IV or ET certification recognized by the state(s).
• Minimum of one (1) year of recent Home Health or Hospice experience.
• Possess and maintain a valid driver’s license.

Responsibilities

• Provide skilled, compassionate nursing care per the nursing process, physician orders, and the patient’s individualized Plan of Care (POC).
• Assess and monitor patient condition, regularly re-evaluating needs.
• Administer medications, treatments, and specialized procedures within scope of practice, training, and demonstrated competency.
• Communicate changes in patient condition and care needs with physicians, supervisors, patients, and families/caregivers to promote continuity of care.
• Educate patients and families/caregivers on medications, treatment plans, disease management, safety, and self-care to support independence.
• Complete accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
• Promote a safe, efficient care environment while supporting activities of daily living, appropriately delegating/supervising staff, and maintaining professional knowledge through ongoing education.
