Job summary

VitalCaring Group is seeking a PRN Weekend Home Health Registered Nurse to lead Start of Care and transitions home for patients over the weekend. You will operate with clinical autonomy, manage plans of care, and ensure high-quality outcomes in a home-health setting.

Ideal candidates bring active RN licensure (or compact eligibility), CPR, BSN, and 1+ year of relevant RN experience, with strong clinical judgment and solid EMR skills. A reliable vehicle and strong communication are essential.

Qualifications

• RN license must be active or have compact eligibility.
• BSN required or preferred.
• CPR certification required.
• Minimum 1 year RN experience in Home Health, Med-Surg, ICU, or Acute Care.
• Strong clinical judgment and ability to work independently.
• Excellent communication and patient-centered approach.
• Proficient with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate status and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including meds, treatments, and interventions.
• Serve as case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions as conditions change.
• Educate patients and families to support disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed and participate in on-call rotation.
