Job summary

VitalCaring Group is seeking a PRN Weekend Home Health Registered Nurse to lead Starts of Care and weekend transitions home in a field-based, autonomous role. You will assess, plan, and execute care for patients in their homes, delivering skilled nursing and education with a focus on reducing hospitalizations and improving recovery.

You will coordinate across physicians, therapists, and aides, maintain clear documentation, and participate in on-call rotation.

Qualifications

Active RN license in Oklahoma
CPR certification required
Graduate of an accredited nursing program
Minimum 1 year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred)
Proficiency with EMR systems and strong clinical judgment

Responsibilities

Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
Develop and manage individualized care plans with physicians and care teams
Deliver skilled nursing care, including medication management, treatments, and interventions
Serve as the case manager, coordinating care across therapists, aides, and physicians
Monitor patient progress and make clinical decisions based on changing conditions
Educate patients and families to support disease management and independence
Coordinate care transitions and discharge planning to reduce readmissions
Maintain timely, accurate documentation in the EMR
Supervise and support Home Health Aides as needed
Participate in on-call rotation as appropriate
