Job summary

PruittHealth is seeking a Weekend/Baylor Physical Therapist (PT) for Home Health in New Bern, NC. You will evaluate function, plan and deliver therapeutic interventions in patients' homes and coordinate care with physicians and families.

You must have a state license, graduate from an accredited PT program, and if foreign-trained, credential evaluation. Prior home health experience is preferred. We offer top pay, mileage reimbursement and flexible weekend schedules.

Qualifications

• Must have a valid professional license in State of practice.
• Graduate of an accredited Physical Therapy Program.
• If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist entry level education in the U.S. by a credentials evaluation organization approved by the APTA.

Responsibilities

• Evaluate patient’s level of function and skilled therapy needs.
• Treat patient to relieve pain, develop or restore function, and obtain maximum outcomes.
• Advise and consult family and interdisciplinary team members.
• Document progress and outcomes using required SOC/Admissions/recertifications and related forms.
• Collaborate with the team to develop and manage the care plan of care.
