Job summary

Pathwell Health seeks a Physical Therapist to cover on-call weekends in Connecticut. Responsibilities include assessing health problems, developing and implementing care plans, and maintaining medical records.

On-call stipend plus pay-per-visit is provided; CT licensure is required. The PT will cover Greenwich, Stratford, New Haven and surrounding areas, delivering direct patient care and documenting progress.

Qualifications

• Graduation from an approved Physical Therapy curriculum.
• Current and valid Connecticut Physical Therapist license.
• Driver’s License, Car Insurance, Car Registration.
• Bilingual Spanish preferred.
• Strong clinical assessment, organizational and communication skills.
• Ability to utilize computers for timekeeping and documentation.
• Recent home health experience preferred.

Responsibilities

• Develops and adjusts a conditioning/rehabilitation plan of care with client, caregiver and physician.
• Tests and screens the client's physical strength to aid physician evaluation.
• Educates client and family on exercise programs and procedures.
• Documents observations, interventions and evaluations in patient records.
• Provide direct patient care as prescribed including treatment planning.
• Review referrals and medical records to determine required PT.
• Record prognosis, treatment and progress in patient chart.
• Identify goals and reevaluation plans.
• Evaluate treatment effects and adjust therapies.
• Participate in service educational activities and clinical studies.
• Perform other duties as assigned.

Job description

The PT would be on call from Friday at 4:30 pm until Monday at 8:00 am (eastern). The PT would be responsible to cover any as-needed visits that arise, as well as scheduled weekend visits including admission visits. Compensation will include an on-call stipend PLUS pay per visit for each visit completed. The Therapist will be responsible for assessing patient health problems and needs, developing and implementing Therapist care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required. The PT would be covering the areas of Greenwich, Stratford, New Haven, and surrounding areas.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential functions include, but are not limited to the following:

• Develops, implements and adjusts a conditioning/rehabilitation plan of care in partnership with the client, representative, caregiver(s) and physician(s)
• Tests and screens the client's physical strength to assist the physician in evaluating the patient's level of function
• Educates and instructs client, family members or other client representatives, about exercise programs and therapeutic procedures to be continued by client
• Accurately documents observations, interventions and evaluations pertaining to client care management and services provided
• Give direct patient care as prescribed including evaluation and treatment planning
• Review physician's referral and patient's medical records to help determine diagnosis and physical therapy treatment required
• Record prognosis, treatment, response, and progress in patient's chart or enter information into computer
• Identify and document goals, anticipated progress, and plans for reevaluation
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Participate in educational activities of the service
• Participate in clinical studies
• Perform other duties as assigned

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)

• Graduation from an approved Physical Therapy curriculum
• Current and valid Connecticut Physical Therapist license (required)
• Recent work experience in home health (preferred)
• Driver’s License,Car Insurance,Car Registration
• Strong clinical assessment, organizational and communication skills
• Ability to handle multiple assignments simultaneously
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation
• Bilingual (Spanish) preferred
• Strong interpersonal skills and good judgement
• Must have the ability to work well under pressure and stay calm and focused in emergent situations
• Be highly self-motivated and results-driven
• A desire for further education to keep up with the ever changing medical advances
