Pathwellhealth in Stamford, CT is seeking a Physical Therapist to assess, plan, organize, and participate in rehabilitative programs that improve mobility and reduce pain. You will collaborate with clients, caregivers, and physicians to tailor a plan of care and deliver direct patient treatment.

The role emphasizes accurate documentation, patient education, and ongoing evaluation of progress. A license, CT registration, and driving credentials are required; bilingual Spanish is a plus.

Qualificações

• Must hold a CT Physical Therapist license
• Graduation from an approved Physical Therapy curriculum
• Experience in home health preferred
• Must have valid driver's license and auto insurance

Responsabilidades

• Develops, implements, and adjusts a rehabilitation plan of care in partnership with the client and physician
• Tests and screens patient strength to assist evaluation of function
• Educates client and family about exercise programs and procedures
• Documents observations, interventions, and evaluations accurately
• Provides direct patient care as prescribed
• Reviews physician referrals and medical records to determine needed treatment
• Records prognosis, treatment, and progress in chart or electronic system
• Identifies goals and plans for reevaluation
• Monitors treatment effects and adjusts accordingly
• Participates in service education and clinical studies
• Performs other duties as assigned
