Job summary

Apploi is seeking a part-time Physical Therapist to cover on-call weekend visits in Riverside, CA. The PT will assess patients, develop and implement care plans, and maintain medical records, including charting prognosis and progress.

The role requires CA license and on-call weekend availability. Responsibilities include creating and adjusting plans of care, documenting interventions, coordinating with physicians and caregivers, and supporting health maintenance and prevention efforts.

Qualifications

• Graduation from an approved Physical Therapy curriculum.
• Current California Physical Therapist license.
• Prior home health experience preferred.
• Driver’s license and auto insurance.
• Bilingual in Spanish preferred.
• Strong assessment and communication skills.

Responsibilities

• Develops and adjusts a plan of care with clients and physicians.
• Screens and assesses patient strength and function.
• Educates clients and caregivers on exercises and rehab.
• Documents interventions and patient progress.
• Provide direct patient care as prescribed.
• Maintain medical records and charts.
