Job summary

Beaumont Home Health and Hospice in Detroit area is seeking a Physical Therapist I to provide skilled home health services ordered by the attending physician. You will educate patients and families and ensure safety, while maintaining accurate documentation.

You will perform OASIS assessments, develop and adjust plans of care, supervise PTAs, and coordinate with physicians and the care team. A valid PT license in the state and driving is required; on-call weekends apply.

Qualifications

• Must be a licensed Physical Therapist in the state of employment.
• Minimum of one year of PT experience.
• Home health experience desired but not required.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled physical therapy in patient homes as ordered by the physician.
• Observe and report patient progress to physician and agency staff.
• Perform OASIS assessments to develop and adjust care plans.
• Develop and implement plan of care and educate patients/families.
• Supervise PTAs and home health aides when appropriate.
• Complete documentation per Medicare/Federal guidelines and agency protocol.
• Ensure safety and promote continuity of care during admissions/transfers/discharges.
