Job summary

HealthPro Heritage is seeking a Weekend Home Health Physical Therapist in Wallingford, CT and surrounding areas. The role features a full-time schedule from Saturday through Tuesday, serving adult patients in home health settings and in locations like Ashlar Village.

Qualifications

• Strong clinical assessment and diagnostic skills.
• Proficient in developing and implementing effective treatment plans.
• Excellent communication and interpersonal skills with a patient-centered approach.

Responsibilities

• Evaluate patients' physical conditions, including mobility, strength, and flexibility.
• Provide therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain; educate patients and families on exercises and home routines.
• Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
• Collaborate with healthcare providers and participate in multidisciplinary team meetings.
