Job summary

Beacon Home Health is seeking a Weekend Licensed Practical Nurse (LPN) to join our Home Health team in Cleveland, TN. You will provide skilled nursing care in patients' homes, following the plan of care and coordinating with the health care team.

The role requires one year of LPN experience, an active LPN license in TN, CPR certification, and reliable transportation to visit multiple residences daily.

Qualifications

• One year of clinical LPN experience.
• Current LPN license in the state of service.
• Current CPR certification.
• Driver's license, vehicle insurance, and access to dependable transportation.
• Ability to reliably commute to multiple patient residences daily within the geographic area.

Responsibilities

• Provide skilled nursing services within the scope defined by state laws and plan of care.
• Instruct family in patient care and maintain a healthy home environment.
• Observe, document, and report patient response to treatment to supervising nurse or physician.
• Collaborate with physicians and RNs, document care promptly, and follow the plan of care.

POSITION OVERVIEW:

The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home. A few position highlights include:

• Provide services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
• Instruct the patient’s family in the care of the patient and maintains a healthy environment for the patient.
• Observe, record, and report to the supervising nurse and/or physician the patient’s response to treatment, and changes in the patient’s condition daily.
• Collaborate closely with physicians and RNs, documents care accurately and promptly and may not alter the plan of care.

THE MUST HAVES:

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification, driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
