Job summary

Beacon Home Health in Cleveland, TN is seeking a Weekend LPN to join our home health team on a PRN basis. You will provide skilled nursing care in patients' homes and coordinate with the health care team.

The role requires one year of LPN experience, an active TN LPN license, CPR certification, and reliable transportation to visit multiple patient residences weekly.

Qualifications

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification.
• Driver's License and vehicle insurance.
• Access to a dependable vehicle or public transportation.
• Must be able to commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Provide services within the scope of practice as defined by state laws and plan of care, coordinating with the health care team.
• Instruct the patient’s family in the care of the patient and maintain a healthy home environment.
• Observe, record, and report patient responses to treatment and changes in condition to the supervising nurse/physician.
