Qualifications

• A minimum of one year of recent, verifiable clinical experience is preferred.
• Prior home care experience is strongly preferred.
• Ability to travel to assigned cases.

Responsibilities

• Follow a designated care plan in accordance with patient's needs.
• Make home visits to clients in assigned geographical areas.
• Monitor clients' conditions and report changes.
• Accurately document observations and evaluations in EMR.

Current Registered Nurse license in OH
