Qualifications

• Registered nursing license issued by the State of Illinois.

Responsibilities

• Performs a comprehensive assessment on assigned patients and documents findings.
• Communicates changes in patient condition promptly to the care team.
• Develops and updates the plan of care with realistic, measurable goals.
• Identifies patient needs and coordinates discharge planning and referrals as needed.
• Provides patient/family education and uses teach-back to ensure understanding.
• Administers medications safely and adheres to unit safety protocols.
