Job summary

Mental Health Association of Rochester is seeking a Certified Nurse Assistant (CNA) to provide in-home care to clients in their residences. The role includes direct patient care, assistance with hygiene and mobility, meal prep, companionship, and light housekeeping.

Travel within the designated area is required; hours are not guaranteed. You will follow care plans and document care using EVV and timesheets to keep accurate records.

Qualifications

• Current DFS certification.
• High school diploma or GED preferred.
• Excellent communication skills, including reading and writing.
• Physical ability to lift and stand for prolonged periods.
• Compassionate approach to patient care.

Responsibilities

• Provide direct patient care including personal hygiene and basic nursing tasks as directed.
• Assist with transfers, repositioning and safe ambulation.
• Prepare meals accommodating dietary needs and preferences.
• Offer companionship and engage clients in activities they enjoy.
• Perform light housekeeping and document care provided.
• Monitor changes in condition and report to supervisor per care plan.
• Use basic medical equipment and log tasks via EVV app or timesheet.
• Maintain patient confidentiality and safety, escalating concerns as needed.
• Encourage patient independence within their abilities.
