Job summary

PAM Health Rehabilitation Hospital of Surprise is seeking a Clinical Navigator for weekend coverage to drive census growth through proactive referral development and clinical assessments. You will build strong referral networks and guide patients through admissions, collaborating with the admissions team and clinicians to ensure a smooth process.

The role supports physicians, case managers, and partner facilities, delivering patient-centered care while representing PAM Health with integrity.

Qualifications

• Degree in a health-related field is preferred.
• Current, unrestricted licensure RN/RT/PT/OT/SLP required/preferred.

Responsibilities

• Proactively develop new referral relationships and expand partnerships within assigned markets.
• Serve as a visible presence in key referral territories by calling on physicians, hospitals, case managers, discharge planners, SNFs, and other partners.
• Identify and pursue new admission opportunities to support census growth.
• Build and maintain relationships with physicians, case managers, discharge planners, ICU and floor nurses, payer representatives, patients, and families.
• Conduct clinical assessments to determine patient suitability for admission.
• Analyze referral and admission trends to identify market growth opportunities and outcomes.
• Partner with admissions and clinical teams for a seamless referral/admission experience.
• Educate referral sources on PAM Health's services, outcomes, and patient success stories.
• Provide recommendations to improve customer service, referral conversion, patient satisfaction, and quality initiatives.
• Represent PAM Health with professionalism and patient-centered approach.
