Job summary

Community Health Resources (CHR) in Coventry, CT, is seeking a part-time Case Manager for the Hendee Road House program. The position offers weekend hours (Saturday-Sunday 8am–4pm) with on-site work supporting adults with mental health needs.

Responsibilities include client assessment, case management, crisis intervention, documentation, and coordination with treatment teams. Bachelor’s degree in human services preferred, plus two years of relevant experience; valid driver’s license required.

Qualifications

• Bachelor’s degree in human services preferred, or education plus equivalent work experience required.
• At least two (2) years’ experience in a mental health setting preferred.
• Valid driver’s license.

Responsibilities

• Uses appropriate therapeutic models to aid clients in understanding and managing their symptoms in a variety of environments.
• Conducts program specific assessments and screens as required by program.
• Provides case management and works in close collaboration with the client’s natural supports and treatment teams.
• Documents all services and contacts with clients, collateral contacts and appointments in compliance with agency standards.
• Transports clients as required by program and participates in staff meetings and trainings.
