Community Health Resources (CHR) in Coventry, CT is seeking a part-time Case Manager to support residents at Hendee Road House. The role focuses on symptom management, crisis intervention, and connecting clients to community resources while collaborating with treatment teams.

Qualifications

• Bachelor’s degree in human services preferred, or education plus equivalent work experience.
• At least two (2) years’ experience in a mental health setting preferred.
• Valid driver’s license.

Responsibilities

• Utilizes appropriate therapeutic models to aid clients in understanding and managing their symptoms.
• Conducts program specific assessments and screens as required by program.
• Provides case management in collaboration with client supports and treatment teams.
• Demonstrates competency with age-related or specialty needs of the population served.
• Documents services and contacts in compliance with agency standards.
• Transports clients as required by program.
• Participates in staff meetings, supervision, and training.
• Additional duties as required.
