Community Health Resources (CHR) is seeking a Part-time Case Manager for the Hendee Road House in Coventry, CT. The role focuses on assisting men with severe mental illness transitioning from long-term hospitalization, providing 24/7 support in residence and community settings.

Responsibilities include symptom management, crisis intervention, and collaboration with treatment teams. Qualifications include a Bachelor’s degree in human services (or equivalent experience) and at least two years’

Qualifications

• Bachelor’s degree in human services preferred, or education plus equivalent work experience required.
• At least two years’ experience in a mental health setting preferred.
• Valid driver’s license required.

Responsibilities

• Uses therapeutic models to aid clients in understanding and managing symptoms.
• Performs program-specific assessments and screens as required.
• Provides case management in collaboration with client supports and treatment teams.
• Manages crisis intervention and de-escalation as needed.
• Documents services and contacts in compliance with agency standards.
• Transports clients as required by program.
• Attends staff meetings, supervision, training, and other meetings as assigned.
