Qualifications

• Master’s degree in Social Work or related field required.
• Two years of related experience.
• CPR certification and Basic Life Support required prior to patient interaction.
• Certification or licensure by Arizona State Board preferred.

Responsibilities

• Develops social histories and psychosocial assessments to inform treatment plans.
• Orients patients and families to the facility and available services.
• Involves patients and families in treatment planning with measurable goals.
• Monitors progress and updates the treatment team on needs and barriers.
• Completes timely EMR documentation and coordinates discharge planning.
