Job summary
Empath Home Health, a member of Empath Health, seeks a compassionate Weekend Baylor RN Registered Nurse to deliver home health care in Lee County (Fort Myers, Bonita Springs, Estero and nearby areas). You will perform comprehensive patient assessments, develop plans of care, and educate families while collaborating with the care team to provide Full Life Care.
The role requires Florida RN licensure, a graduate of an approved nursing program, and at least one year of direct patient care; BSN/MSN
Qualifications
- Graduate of an approved nursing program and current Florida RN license.
- BSN or MSN preferred.
- Minimum of one year direct patient care experience; home health RN experience preferred.
Responsibilities
- Perform comprehensive patient assessments including required OASIS assessments.
- Develop and revise individualized plans of care.
- Deliver skilled nursing services per plan of care.
- Counsel and educate patients and families on disease processes and self-care.
- Collaborate with the interdisciplinary team to ensure holistic care.
