The ideal candidate holds an active Florida RN license with Home Health experience, plus strong assessment and care-planning skills.

Qualifications

• Active Florida RN license required.
• Home Health RN experience preferred.
• Minimum 1 year direct patient care experience.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments.
• Develop, initiate, and revise plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments per plan of care.
• Counsel and educate patients and families on disease processes and prevention.
