Job summary

Optum is seeking a Registered Nurse for a Baylor Home Health weekend position with a 32-hour salaried schedule and full-time pay and benefits. The role offers a flexible weekend option (Friday–Sunday or Saturday–Monday) and includes a $5,000 sign-on bonus.

The RN will provide and coordinate home health nursing care, perform comprehensive assessments, and collaborate with physicians and the care team to ensure continuity of care in a patient’s home setting.

Qualifications

• Current RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred; ability to obtain within 90 days of hire.
• 1+ year of clinical experience.

Responsibilities

• Develops and updates patient plans of care in collaboration with physicians and the interdisciplinary team.
• Provides and coordinates high-quality clinical care; supervises home health aides and LPN/LVN staff.
• Documents patient care per policy and payer requirements; communicates with physicians, patients, families, and staff.
• Participates in orientation and precepting of new staff; supports quality improvement initiatives.
