Choose from Friday-Sunday or Saturday-Monday shifts and join a team dedicated to high-quality home health care. You will coordinate care, perform assessments, and collaborate with physicians and the interdisciplinary team in patients' homes.

Qualifications

• Current RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience.

Responsibilities

• Develops, implements, and updates patient plans of care with physicians and the team.
• Provides and coordinates high‑quality clinical care and supervises aides and LPN/LVNs.
• Documents all patient care per policy and payer requirements; communicates with physician, patients, and families.
• Participates in orientation and precepting; contributes to quality and safety initiatives.
• Demonstrates professionalism and participates in on‑call/weekend rotations.
• Functionally supports diverse patient populations and care settings.
