Job summary

Pathways Healthcare is a leading home health and hospice organization delivering patient-centered care in the home setting. The RN positions focus on preventive and rehabilitative nursing, medication administration, and care plan management across Cranston and surrounding areas.

The role emphasizes strong clinical judgment, compassionate bedside care, and collaboration with physicians and the interdisciplinary team to create robust plans of care. Westwood-based office with field responsibilities.

Qualifications

• Nursing degree from an accredited institution.
• Preferably 2+ years of nursing experience focused on geriatrics.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.

• Salary: Commensurate with Experience
• Main Office location: Westwood, MA
• Service Area: Cranston, Fall River and surrounding territories
