Job summary

Aspire Home Healthcare of Ohio is hiring five full-time RNs to provide one-on-one skilled nursing in patients' homes in Dublin/Columbus area. You’ll coordinate with physicians and care teams to ensure top-quality outcomes for each patient.

Qualifications

Responsibilities

• Develop, implement and evaluate plans of care.
• Administer medications and wound care.
• Educate patients and families on care routines and disease management.
• Collaborate with physicians, therapists and the care team.
• Document care in EMR and ensure Medicare/Medicaid compliance.

Position Summary

Key Responsibilities

• Conduct initial and ongoing assessments — including Start of Care (SOC), Recertifications, Resumptions, and Discharges
• Develop, implement, and evaluate individualized plans of care
• Administer medications, wound care, and treatments
• Educate patients and families on care routines, medications, and disease management
• Collaborate closely with physicians, therapists, and the interdisciplinary team
• Document care accurately in WellSky EMR (or similar)
• Ensure compliance with Medicare, Medicaid, and agency standards
• Participate in weekend rotation as needed

Qualifications

• 2+ years of home health nursing experience
• Skilled in managing diverse home health diagnoses
