Job summary

Vantage Care Management seeks a confident, compassionate Weekend Admissions Liaison to join our team. You will serve as the primary weekend contact, guiding families through the admissions journey with clear communication and clinical insight.

This role requires regulatory knowledge to support safe, compliant admissions, collaboration with nursing and administrative teams, and the ability to manage busy weekend workflows with professionalism and empathy.

Qualifications

• Must be a licensed professional (RN, LPN, or Licensed Social Worker).
• Experience in admissions within healthcare or long-term care is preferred.
• Demonstrated PASRR knowledge and ability to complete screenings independently.

Responsibilities

• Manage weekend inquiries and guide families through admissions.
• Collaborate with nursing, rehabilitation, and administrative teams to confirm clinical eligibility.
• Complete admission paperwork and collect all required documentation.
• Ensure PASRR screenings are completed accurately and timely before admission.
• Maintain clear communication with hospitals, families, and facility staff.
• Represent Vantage Care with professionalism and compassion.
