Job summary

21st Century Home Health Services (21HHS) seeks a part-time Registered Nurse Case Manager to cover the Gilroy/Morgan Hill area. You will perform SOC admissions, follow-up visits, and home visits on Saturdays and Sundays, coordinating with the clinical team to deliver high-quality home health care.

Strong OASIS experience and IV therapy skills are highly valued, along with CA RN licensure and valid driver’s license.

Qualifications

• Current and unrestricted CA RN license required.
• BLS/CPR certification required.
• Experience with Start of Care (SOC) and OASIS is highly desired.
• Valid CA Driver's License.
• Ability to work in homes within Gilroy/Morgan Hill territory.

Responsibilities

• Complete initial patient assessments to determine home care needs.
• Conduct SOC admissions and ongoing visits within territory.
• Perform wound care, wound treatments and IV infusions as ordered.
• Document observations, interventions, and patient responses accurately.
• Coordinate with physicians and the clinical team for optimal care.
• Provide patient and family education and support.
• Travel to patient homes with flexible Saturday/Sunday schedule.

Part time Registered Nurse Case Manager

Anticipated Territory : Gilroy/Morgan Hill

Schedule: Saturday/Sunday

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Concierge for Case Managers: Dedicated resources assist with time consuming administrative tasks
• Flexible work schedules: Create your own schedule and hours
• Targeted territories. Typically patient visits are within a 5-15 mile radius.
• Autonomy: No clocking in/out
