Job summary

Empath Health is seeking a registered nurse with hospice leadership experience to lead the Access Team within the Admission-Care Navigation Department. You will oversee daily operations, ensure alignment with the organization's mission, and coordinate patient access processes across multiple locations.

The role emphasizes regulatory compliance, staff coaching, and performance feedback to sustain high-quality Full Life Care. Travel between locations is required as part of the duties.

Qualifications

• RN licensure required.
• Two years hospice experience with at least two years of management experience preferred.
• At least five years total experience as an RN preferred.

What You'll Need

• Education and/or Experience: A.S. degree required - Bachelor's degree (B.A.) from four-year college or university and Master's degree preferred.
• Two years Hospice experience with at least two years of management experience preferred.
• At least five years total years experience as an RN preferred.
• Licensure: RN
• Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file. This position requires travel between multiple locations.
