Qualifications

• Active RN license required.
• Minimum of two years nursing experience preferred.
• Valid driver’s license with ability to travel locally between residential locations.
• Proficient with electronic medical records and computer systems.

Responsibilities

• Assess admissions and complete ongoing nursing assessments.
• Administer medications, vaccinations, and physician-ordered treatments.
• Monitor health conditions, infections, and chronic medical needs.
• Coordinate medical, psychiatric, dental, and specialty appointments; provide diabetic foot assessments and first aid as needed.
• Triage medical concerns and communicate with physicians and psychiatrists.
• Educate DSPs on medication administration, infection prevention, and proper use of medical equipment.
• Conduct medication room, eMAR, and EHR audits.
• Assist with care planning and policy development.
• Support infection prevention initiatives and health education.
• Ensure HIPAA, state regulations, and company policies compliance.
• Serve as a clinical resource to interdisciplinary teams.
