Job summary

Responsibilities include patient assessment, medication administration, charting, discharge planning support, infection control, and collaboration with families and physicians to maintain high quality care.

Qualifications

• Must be a Licensed Practical Nurse (LPN) in good standing and licensed by Oklahoma.
• Must be at least 18 years of age and pass a background check.
• Minimum 1 year experience in Skilled Nursing Facility or Long-Term Care.
• Diploma from an accredited School of Nursing is required.
• Ability to read, write, and follow directions.
• Must relate well to residents and families and work cooperatively.
• Must maintain regular attendance.
• Excellent communication and teaching skills in individual and group settings.

Responsibilities

• Provide direct nursing care to residents.
• Supervise CNAs and CMAs in daily activities.
• Maintain QA program for the nursing department.
• Record diet orders and communicate with dietary.
• Chart communications with physicians and families.
• Transcribe orders and document in medical records.
• Assist with discharge planning and follow-ups.
• Take and record vital signs and ensure charting accuracy.
• Maintain confidentiality and protect residents from neglect or abuse.
