Job summary

Ecumen is seeking a Hospice Social Worker (PRN/Casual) for the Duluth and North Branch teams. Flexible weekday schedule from 8am to 4:30pm with no weekends and no holidays.

The role focuses on direct psychosocial support for hospice patients and families, with strong collaboration in an interdisciplinary team. Responsibilities include psychosocial assessments, bereavement support, care planning, and coordination of resources for end-of-life care.

Qualifications

• Master's degree in social work required.
• Licensed Social Worker in good standing required in Minnesota.
• Two or more years of social work experience in a health care setting preferred.

Responsibilities

• Assess psychosocial status of terminally ill patients and families; provide resources and bereavement support.
• Assess family/caregiver bereavement risk and provide appropriate resources.
• Monitor family dynamics and caregiver safety; plan interventions as needed.
• Develop individualized care plans with patient and family; attend interdisciplinary meetings.
• Assist with funeral planning and resources for financial, legal, and healthcare decisions.
• Lead care conferences with hospice team and family; provide bereavement follow-up.
