Job summary

Qualifications

• BA degree in human services or related behavioral sciences.
• One year of experience working with ID/DD individuals (volunteering or having a relation).
• OR one year of case management experience.

Responsibilities

• Conduct annual assessment and person-centered service plan.
• Conduct monthly face to face visits with participants.
• Assist clients with accessing community resources.
• Monitor participants' progress with waiver services.
• Assist with building a network of supports.
• Ensure participant choice and access to community resources.
