Job summary

The Ole Homeplace is seeking a compassionate Human Services Case Manager to support Participants in Kentucky Waiver Programs, coordinating access to needed services and developing individualized care plans. You will travel locally to conduct in-person visits and appointments, building strong relationships with families and service providers.

Ideal candidates hold a relevant degree (or equivalent experience) and demonstrate trauma-informed care, professional boundaries, and person-centered

Qualifications

• Bachelor’s degree in Social Work, Human Services or related field is required.
• Alternate: Bachelor’s in any field with 1+ year Human Services experience.
• Associate degree in Behavioral or Social Science with 2+ years Human Services experience.
• Kentucky RN with 2+ years aging/disabilities experience.
• Master’s degree in Health or Human Services is acceptable.

Responsibilities

• Act as central contact and support for Participants in Kentucky Waiver Programs.
• Create and manage personalized care plans aligned with participant goals.
• Build strong relationships with participants, families, and providers.
• Complete required training (trauma-informed care, reporting, boundaries, person-centered thinking, incident reporting).
• Engage in local travel (about 50%) for in-person visits and appointments.

What You'll Do:

• Act as a central point of contact and support for Participants in Kentucky Waiver Programs, ensuring they have access to the resources and services they need.
• Create and manage personalized care plans that are respectful of each participant's goals and preferences.
• Build strong, positive relationships with participants, their families, and a network of service providers.
• Complete required training to maintain a high level of competency and compliance, including topics like trauma-informed care, abuse and neglect reporting, professional boundaries, and person-centered thinking, and incident reporting.
• Engage in local travel (approximately 50%) to conduct in-person visits and attend necessary appointments.

Requirements

Who We're Looking For: You should possess one of the following qualifications:

• A Bachelor's degree in Social Work, Human Services, or a related field.
• A Bachelor's degree in any field, complemented by at least one year of Human Services experience.
• An Associate degree in a Behavioral or Social Science field, with two years of Human Services experience.
• A Kentucky-licensed RN with two or more years of experience in the field of aging or disabilities.
• A Master’s degree in a Health or Human Services field from an accredited college or university.

Relevant degrees include Rehabilitation, Nursing, Counseling, Education, Gerontology, Human Services, and Sociology.
