Job summary

Residential Transitions, Inc. (RTI) in Minnesota seeks a Waiver Case Manager to provide person-centered case management for CADI/BI waiver services in Hennepin County.

You will manage a caseload, visit clients regularly, coordinate intakes and discharges, collaborate with physicians and teams, ensure documentation and contracts, and help clients access community resources. In-person training at the main office is required for the first 60 days.

Qualifications

  • Graduate from a four-year college with a major in social work, psychology, sociology or closely related field, OR
  • Graduate from a four-year college with any major and one year of experience as a social worker/case manager in a social service agency.

Responsibilities

  • Manage and maintain a caseload of individuals with serious mental illness, substance use disorders and co-occurring conditions.
  • Visit clients at least once a month and twice annually per waiver rules.
  • Coordinate client intake and discharges within set parameters and timelines.
  • Collaborate with physicians, mental health providers, and other teams to develop and update plans for each client.
  • Ensure financial authorizations and provider contracts are in place.
  • Document services in individual records to track results and ensure standards.
  • Access community and housing resources to match needs and preferences.
  • Provide crisis assessment and intervention to keep clients safe.

Position Overview:

Residential Transitions Inc. (RTI) is seeking a dedicated Waiver Case Manager forHennepin County. We work with people integrated into the community, living in their own homes and in our residential settings. Our mission is to help people improve their quality of life by empowering them with knowledge, resources and skills and treating them with dignity and respect. Waiver case manager provides person-centered case management services to individuals diagnosed with serious and persistent mental illness, substance use disorders and co-occurring medical conditions who are eligible to receive services through the Community Access for Disability Inclusion (CADI) or Brain Injury (BI) Waiver Case Management services through Hennepin County. This position requires in-person training at our main office for the first 60 days.

Role Responsibilities:

  • Manage and maintain a caseload of individuals who are diagnosed with serious and persistent mental illness, substance use disorders and co-occurring medical conditions throughout the respective County
  • Visit clients at least once a month and twice annually in accordance with waiver-specific rules.
  • Coordinate client intake and discharges, within set parameters and timelines.
  • Collaborate with other professionals such as physicians, mental health providers, and other teams as needed to develop and update plans for each client.
  • Ensure financial authorizations and provider contracts are in place
  • Ensure services are documented in individual records so information is readily available, results of services can be determined, and record-keeping standards are met.
  • Access available community and housing resources so that necessary and relevant resources match an individual’s needs and preferences.
  • Provide crisis assessment and intervention so individuals receive the assistance they need to be safe in their home and community.
Required Qualification and Experience:
  • The Waiver Case Manager shall be qualified under Minnesota law as follows:
    • Be a graduate from an accredited four-year college with a major in social work, psychology, sociology or a closely related field. OR
    • Be a graduate from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
  • Willingness to travel within the Metro Area, for applicable mileage reimbursements, with a current driver’s license, clean record, and personal vehicle with insurance; and able to transport clients in that vehicle.
  • Must not be actively receiving services from Residential Transitions Inc. and must not have received services within the last two years.
Preferred Qualification and Experience:
  • Master's degree in behavioral science or related field
  • Experience providing direct services to adults with serious and persistent mental illness
  • Experience with case management and commitment proceedings

Compensation: $50,000.00 - $55,000.00 per year
