Job summary

Residential Transitions Inc. (RTI) is seeking a Waiver Case Manager in Hennepin County, MN. This role involves person-centered case management for CADI/BI waiver services, with in-person training at the main office for the first 60 days.

You will manage caseloads, visit clients monthly, coordinate intakes/discharges, and collaborate with medical and care teams to create customized plans. Hybrid work available.

Qualifications

  • Graduate from an accredited four-year college with a major in social work, psychology, sociology or related field.
  • Graduate from an accredited four-year college with any field and one year of social worker/case manager experience.
  • Willingness to travel within the Metro Area with a valid driver's license and vehicle.
  • Must not be actively receiving services from Residential Transitions Inc. and must not have received services within the last two years.

Responsibilities

  • Manage and maintain a caseload of individuals with serious mental illness and co‑occurring conditions in the county.
  • Visit clients at least monthly and twice annually per waiver rules.
  • Coordinate client intake and discharges within set parameters and timelines.
  • Collaborate with physicians, mental health providers, and teams to update care plans.
  • Ensure financial authorizations and provider contracts are in place.
  • Document services in individual records to support data and compliance.
  • Access community resources to match needs and preferences.
  • Provide crisis assessment and intervention as needed.

Position Overview

Residential Transitions Inc. (RTI) is seeking a dedicated Waiver Case Manager for Hennepin County. We work with people integrated into the community, living in their own homes and in our residential settings. Our mission is to help people improve their quality of life by empowering them with knowledge, resources and skills and treating them with dignity and respect. Waiver case manager provides person-centered case management services to individuals diagnosed with serious and persistent mental illness, substance use disorders and co-occurring medical conditions who are eligible to receive services through the Community Access for Disability Inclusion (CADI) or Brain Injury (BI) Waiver Case Management services through Hennepin County. This position requires in-person training at our main office for the first 60 days.

Role Responsibilities
  • Manage and maintain a caseload of individuals who are diagnosed with serious and persistent mental illness, substance use disorders and co-occurring medical conditions throughout the respective County
  • Visit clients at least once a month and twice annually in accordance with waiver-specific rules.
  • Coordinate client intake and discharges, within set parameters and timelines.
  • Collaborate with other professionals such as physicians, mental health providers, and other teams as needed to develop and update plans for each client.
  • Ensure financial authorizations and provider contracts are in place
  • Ensure services are documented in individual records so information is readily available, results of services can be determined, and record-keeping standards are met.
  • Access available community and housing resources so that necessary and relevant resources match an individual’s needs and preferences.
  • Provide crisis assessment and intervention so individuals receive the assistance they need to be safe in their home and community.
Required Qualification and Experience
  • The Waiver Case Manager shall be qualified under Minnesota law as follows:
    • Be a graduate from an accredited four-year college with a major in social work, psychology, sociology or a closely related field. OR
    • Be a graduate from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
  • Willingness to travel within the Metro Area, for applicable mileage reimbursements, with a current driver’s license, clean record, and personal vehicle with insurance; and able to transport clients in that vehicle.
  • Must not be actively receiving services from Residential Transitions Inc. and must not have received services within the last two years.
Preferred Qualification and Experience
  • Master's degree in behavioral science or related field
  • Experience providing direct services to adults with serious and persistent mental illness
  • Experience with case management and commitment proceedings

Physical & Mental Requirements
  • Walk, stand, sit, bend, climb stairs, and perform crisis prevention/intervention techniques
  • Use hands for writing, typing, and data entry
  • Lift up to 40 lbs. and occasionally exert physical effort during duties
  • Maintain vision and hearing necessary for reading charts, operating office equipment, using a computer, and driving a vehicle
