- Provide waiver case management services to adults or minors eligible for the CADI or BI Waiver
- Help individuals obtain and coordinate supportive services addressing social, recreational, health, educational, and vocational needs
- Manage a caseload of no more than 45 individuals
- Meet with each individual on the caseload at least twice annually
- Refer and coordinate waiver-funded services supporting individual needs and goals
- Ensure confidentiality of records and interactions
- Participate in professional meetings, workshops, supervision meetings, and team-based service decisions
- Review and maintain files and complete intake and authorization paperwork on time
- Complete accurate case notes for work completed
- Deliver person-centered, community-based services in homes, schools, and other community locations
- Maintain reliable transportation and professional boundaries
- Develop community relationships and maintain the agency’s public image
- Refer clients to professionals with supervisor approval
- Communicate and coordinate with service providers
- Educate self, individuals, and support teams about community resources
- Research resources and collaborate with referral sources and agencies serving people with cognitive disabilities or mental illness
- Report to a designated supervisor
- Assess changing needs, develop plans with service providers, link individuals to community resources, and monitor service effectiveness
- Participate in other Reach for Resources functions as assigned
Requirements
- Only applicants who meet the DHS qualifications for a CADI/BI Case Manager will be reviewed
- Public health nurse OR Registered nurse OR Social worker qualification
- Social worker qualification: graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field; OR four-year college degree in any field plus one year of experience as a social worker/case manager/care coordinator in a public or private social service agency
- Bachelor's degree in social work, psychology, sociology, or human services
- Knowledge, education, and experience working with individuals with physical disabilities, brain injury, or mental illness
- Skilled in identifying and assessing a wide range of recipient needs
- Knowledge of local community resources, activities, and social services and how to access them
- Ability to work independently, enjoy flexibility, and manage stress effectively
- Able to pass a criminal background check
- Current driver’s license in good standing and own reliable transportation

- Strong time management, organization, and tracking skills
- Ability to communicate, write reports, and complete documentation in a professional, person-centered manner
