Responsibilities

• Provide case management services to individuals receiving CAC, CADI, BI, and DD waiver services to support health, safety, stability, and community integration.
• Conduct initial assessments, reassessments, monitoring visits, and service reviews to identify strengths, needs, risks, and support requirements.
• Complete MNChoice assessments and other required evaluations in accordance with state and federal guidelines.
• Develop, implement, and monitor individualized person-centered support plans based on each individual's goals, preferences, strengths, and assessed needs.
• Authorize waiver services and supports consistent with assessed needs, available resources, and program requirements.
• Coordinate services with providers, families, guardians, healthcare professionals, schools, vocational programs, and community agencies.
• Monitor service delivery to ensure services are provided appropriately, effectively, and in accordance with approved support plans.
• Identify health and safety concerns and take appropriate action to address risks and ensure client well-being.
• Assist individuals and families in accessing community resources, benefits, housing supports, employment services, healthcare services, and other needed supports.
• Maintain accurate, timely, and compliant documentation within SSIS, MMIS, MNChoice, and other required electronic systems.
• Ensure compliance with federal and state waiver regulations, vulnerable adult reporting requirements, mandated reporting laws, and agency policies.
• Respond to changing client circumstances and adjust support plans and services as needed.
• Advocate for individuals to ensure access to appropriate services and supports that promote independence and self-determination.
• Participate in multidisciplinary team meetings and collaborate with service providers to coordinate care and achieve desired outcomes.
• Participate in ongoing training and professional development opportunities.
• Other duties as assigned.

This position is part time, working 24 - 28 hours per week. The hiring range for this position is $37.50 - $41.94 per hour. The full range for this position is $37.50 - $52.46 per hour.

The HCBS Waiver Case Manager plays a vital role in supporting individuals with disabilities, complex medical needs, brain injuries, and other long-term support needs by coordinating Home and Community-Based Services (HCBS) through the Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC), Brain Injury (BI), and Developmental Disabilities (DD) waiver programs. This position is responsible for assessing and reassessing client needs, authorizing and monitoring services, developing person-centered support plans, and coordinating resources that promote health, safety, independence, and quality of life. The HCBS Waiver Case Manager collaborates closely with individuals, families, providers, healthcare professionals, and community agencies to ensure effective service delivery and compliance with federal and state waiver requirements. Strong communication, assessment, problem-solving, and organizational skills are essential, along with a commitment to person-centered and strengths-based practice.

• Bachelor’s degree in Social Work, Nursing, Public Health, Human Services, or a closely related field required.
• Certified Public Health Nurse (PHN), Registered Nurse (RN), or Social Worker preferred.
• Two (2) years of related experience preferred.
• Equivalent combinations of education and experience may be considered.
• Experience with Home and Community-Based Services (HCBS) waiver programs preferred but not required.
• MNChoice Certification may be required based on agency needs.
• Valid driver’s license and reliable transportation required.
• Must pass a criminal background check and meet DHS requirements for working with vulnerable populations.

Physical Requirements
