Job summary

Optum is seeking a Nurse Practitioner or Physician Assistant for statewide in-home visits across Washington. This field-based role focuses on preventive care, patient education, and closing care gaps without prescribing duties.

You will travel throughout Washington to perform assessments, educate patients, and coordinate with PCPs, with a schedule that avoids on-call, weekends, or major holidays.

Qualifications

• NP: Active AANP or ANCC certification.
• PA: Active NCCPA certification.
• Active, unrestricted licensure.
• Valid driver’s license and reliable transportation.
• Ability to travel extensively throughout Washington.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other members.
• Review medical history, medications, vital signs, and perform exams; identify care gaps.
• Educate patients on conditions and treatment plans.
• Address social determinants of health and connect with community resources.
• Recognize urgent concerns and arrange appropriate follow-up care.
