Job summary

UnitedHealth Group is seeking Nurse Practitioners or Physician Assistants for in-home care within Washington state. Roles involve comprehensive in-home assessments, chronic disease management, and coordination with an interdisciplinary team, with travel across the state and no nightly shifts.

Weekly overnight travel may occur, with options for 8-hour or 10-hour days and a Monday–Friday schedule. Compensation includes a $40,000 sign-on or student loan repayment and a robust benefits package.

Qualifications

• Active licensure in the state you will practice in.
• Certification for NP or PA as required by role.
• Access to reliable transportation and a valid driver's license.

Responsibilities

• Perform in-home assessments including medical history, meds reconciliation, vitals and exam.
• Manage acute events or chronic condition exacerbations and coordinate care.
• Support short-term sub-acute rehab needs and document ICD-10 codes.
• Collaborate with interdisciplinary care team and plan advanced care.
• Provide counseling and education to patients and caregivers.
• Travel throughout the state up to 85% and maintain scheduling requirements.
