Job summary

State of Washington is hiring a WA Cares Fund Assessment & Community Support Specialist (Social Service Specialist 2) in Vancouver or Tumwater, WA. The role conducts long-term care assessments, builds personalized plans, and helps eligible residents access earned benefits under the WA Cares Fund.

As part of the Home & Community Living Administration, you will educate clients, collaborate with families and providers, and adapt workflows to support safe community living.

Qualifications

• 12 months as a Social Service Specialist 1 plus completion of the agency's training program within 12 months.
• Master's degree or bachelor's with related social services experience as detailed in the posting.

Responsibilities

• Conduct in-person and remote long-term care functional assessments to determine WA Cares Fund eligibility.
• Develop individualized service plans reflecting clients' needs and goals.
• Educate clients and families about long-term care options, benefits, rates and limitations.
• Provide guidance on provider enrollment, service search, and rate-setting best practices.
• Refer individuals to community resources or alternative programs when WA Cares Fund is not pursued.
