Sea Mar Community Health Centers in Washington state is seeking a WA Cares Beneficiary Assessment Coordinator to coordinate eligibility and assessments for long‑term care beneficiaries. You will conduct in‑depth assessments, assist with applications, and connect clients to local and state programs.

The role requires knowledge of long‑term care and social service networks, strong computer skills, and the ability to work independently in a fast‑paced community health setting.

Qualifications

• BA in social services or related field and two years of paid or volunteer experience providing direct human services.
• Must obtain Basic Life Support (BLS) CPR within 90 days of hire and maintain it.
• Valid WA State Driver’s License and proof of liability insurance.
• Bilingual in Spanish required.

Responsibilities

• Coordinate WA Cares beneficiary eligibility and assessments.
• Assist beneficiaries with applications and referrals to programs.
• Drive to beneficiaries' homes for meetings as needed.
• Maintain records and ensure timely assessments and case documentation.
• Provide information to enable aging in place services and coordinate with Sea Mar resources.
