Job summary

BoldAge PACE in New York is seeking a Vice President of Participant Experience to lead participant-facing service lines including Social Work, Home Health, and therapeutic services. You will drive a patient-centered culture and improve outcomes with a hands-on, collaborative approach.

Reporting to the Chief Clinical Officer, you will oversee POLST, housing, Spend Card, and self-pay/Spend-Down programs while partnering with leaders across operations, quality, and clinical teams to align practice

Qualifications

• Master’s degree/licensure requirements; LCSW preferred.
• Minimum of five years in leadership in geriatric care or related settings.
• Experience with Medicaid spend-down and financial oversight preferred.
• Clinical leadership, mentoring clinicians, and improvement processes preferred.
• PACE program experience strongly preferred.
• Two years of clinical care for medically complex older adults preferred.

Responsibilities

• Review care plans, assessments, and documentation across service lines to ensure gaps are addressed.
• Monitor financials and identify opportunities for cost management (PMPM).
• Provide matrixed clinical leadership for Social Work, Home Health, and care planning.
• Ensure POLST data integrity and alignment with goals of care.
• Administer Spend Card and self-pay/Medicaid spend-down processes.
• Lead quality improvement, audits, and corrective actions across service lines.
