Job summary

Abode Care Partners is seeking a Vice President of Clinical Services for Primary Care, reporting to the Chief Medical Officer. This senior physician executive leads Regional Medical Directors and provides clinical oversight across markets, facilities and service lines to drive productivity, quality, and patient experience.

The VP will shape the Model of Care, integrate value-based strategies, and ensure timely follow-up after hospitalizations.

Qualifications

• Active, unrestricted physician license.
• Current unrestricted DEA registration as appropriate.
• Willingness to obtain licensure in required states.
• Travel 20-40% as needed.

Responsibilities

• Directly manage and develop Regional Medical Directors and establish clear expectations for clinical leadership, provider management, regional execution, and performance.
• Oversee provider performance across productivity, quality and clinical outcomes, avoidable hospitalizations and readmissions.
• Lead Model of Care development, clinical standards, and EMR-enabled workflows across service lines.
• Ensure timely follow-up after hospitalization and effective escalation pathways for care transitions.
