Job summary

CHI Health at Home is seeking a Bereavement and Volunteer Coordinator to support multiple site locations by planning, implementing, and maintaining the volunteer program for patients and caregivers.

The role involves recruiting, training, evaluating, and retaining volunteers; reporting hours; coordinating grief counseling services; and participating in PI and QAPI activities to improve hospice care quality.

Qualifications

  • Bachelor’s degree required in healthcare, human services, or related field.
  • Experience with death/dying issues required.
  • Two years in human services or volunteer coordination.
  • Knowledgeable in using Google Workspace and Gmail.

Responsibilities

  • Coordinate recruitment, training, evaluation, and retention of volunteers.
  • Report monthly volunteer hours and maintain volunteer documentation.
  • Develop bereavement education materials for patients, caregivers and staff.
  • Participate in PI/Quality Improvement initiatives and related committees.
  • Coordinate bereavement services delivery to caregivers and plan group events.

Job Summary and Responsibilities

At CHI Health at Home we strive to embody our mission of delivering hope and healing to those we serve. As the Bereavement and Volunteer Coordinator, you will support the success of our site locations by planning, implementation, and maintenance of the volunteer program in order to meet the needs of patients and caregivers.

You'll work independently according to organizational policies and collaborate with team members to meet site goals. Travel may be required between locations.

  • Coordinates the recruitment, training, evaluation, and retention of volunteers. Updates volunteer curriculum on an ongoing basis.

  • Responsible for accurately reporting monthly volunteer hours, obtaining volunteer documentation and recording activities.

  • Maintains a communication tool for volunteers to stay involved and current on hospice activities.

  • Participates in Performance Improvement (PI) Program Coordination; including assisting with the overall development and implementation of the PI plan, assisting in the identification of goals and related patient outcomes, participation and reporting of activities and outcomes, development and implementation of corrective plans.

  • Participates in Quality Assurance Performance Improvement (QAPI) in hospice activities such as IDT meetings, in-service education, department meetings, and relevant committees.

  • Coordinates the assessment and delivery of grief counseling needs and services in the bereavement program by utilizing professional staff and volunteers as appropriate to ensure timely and appropriate services are provided to caregivers.

  • Plans, implements, and coordinates bereavement group events.

  • Develops educational programs and materials for patients, caregivers, staff and all the community on loss, grief, and coping with bereavement.

  • Coordinates the bereavement mailings.

  • Other duties as assigned

Job Requirements
  • Bachelor’s Degree required in healthcare, human services or related education. Experience may be substituted.

  • Experience and/or training with death / dying issues required

  • Knowledgeable in using Google programs (Docs, Slides, Sheets) and Gmail.

  • Two years in human services, having volunteer or volunteer coordination experience.

Pay Range

$23.69 - $36.72 /hour
