Job summary

Opportunity Enterprises is seeking a Job Coach to deliver on-site job training, mentoring and workplace assessments across LaPorte, Porter and surrounding counties. You will support Vocational Rehab clients and Waiver/Community Support clients with progress tracking and documentation.

You will develop natural supports at worksites, maintain compliance with CARF standards, and collaborate with employers, clients, and families. Flexibility for varied shifts is required.

Qualifications

• High School Diploma is required.
• 1–3 years' experience in related field preferred.
• Experience with developmentally disabled population preferred.
• Excellent written and verbal communication skills.
• Strong organizational and time management abilities.

Responsibilities

• Provide coaching and training to actively working Vocational Rehab clients.
• Provide coaching and on-going training to clients receiving Extended Services through Waiver and Community Support budgets as outlined in their budgets.
• Assist with the development of natural supports at the worksite.
• Monitor and document client progress to maintain goals.
• Maintain documentation daily, weekly, and monthly per department standards.
• Advocate for services to meet individual client needs.
• Maintain productivity of 85% weekly based on caseload.
• Develop relationships with employers, referral sources, clients, and families.
• Attend client conferences and read files prior to coaching services.
• Work across Porter, Lake, and surrounding counties with varied shifts.
