Job Description

We are seeking a physical therapist to make home visits in the Cypress, Texas area. If you enjoy making a difference and desire a flexible schedule, this is an ideal opportunity.

Responsibilities

1. Provide physical therapy services according to physician’s plan of care.
2. Assess and evaluate patients’ therapeutic/rehabilitative/functional status and home environment.
3. Assess muscle strength, mobility, gait, ROM, and rehab potential.
4. Direct physical therapy treatments and instruct patients and caregivers.
5. Determine priority needs and report patient reactions to physicians.
6. Initiate therapy programs, train family/caregivers, and participate in case conferences.
7. Identify additional home health needs and refer as necessary.
8. Prepare clinical summaries and participate in discharge planning.
9. Provide consultation and inservice education as needed.
10. Supervise physical therapy assistants and home health aides.

Qualifications

• Degree from an accredited physical therapy program (bachelor’s or master’s).
• State licensure, if applicable.
• Minimum two years of experience, preferably in home health.
• Good communication and organizational skills.
• Current CPR certification.
• Valid driver’s license and insured vehicle.
