Job summary

Your Health is seeking a Registered Nurse to service patients across Athens and surrounding areas. The Nurse Case Manager visits patients in homes and facilities, coordinating with providers and initiating telemedicine when appropriate.

This full-time role requires reliable transportation and daily travel, 8-hour shifts, typically Monday through Friday. The Nurse will build Care Management plans and document activities to support quality outcomes.

Qualifications

• Must be a licensed nurse in good standing.
• Minimum 3 years clinical experience preferred.
• Experience in community settings preferred.
• Must have reliable transportation and be willing to travel daily.

Responsibilities

• Facilitate receiving medical records from patient’s providers and specialists.
• Review medical records and enroll patients in Care Management.
• Initiate a Care Management Plan of Care if eligible and provide post-discharge education.
• Coordinate care and communicate with the care team; reconcile medications.
• Evaluate for home health, RPM devices, and other resources to improve quality of life.
• On-call may be required.
