Job summary

Your Health Organization is recruiting a Registered Nurse to serve patients throughout the Athens area as a Nurse Case Manager. The role involves home visits, coordinating with providers, and initiating telemedicine visits to support comprehensive care.

Travel to patients’ homes and facilities is required daily. The position is full-time, with 8-hour shifts Monday–Friday (8am–5pm), offering a salary-based compensation package and opportunities for professional growth.

Qualifications

Must be a licensed Registered nurse.
Three years clinical experience preferred.
Experience in community settings preferred.
Proven ability to effectively communicate and collaborate with care teams.
Strong written and verbal skills.
Basic computer knowledge.
Ability to manage multiple responsibilities and demonstrate sound judgment.
Willingness to travel between offices and facilities; must have reliable transportation.

Responsibilities

Facilitate receiving all medical records from the patient’s primary provider and specialists.
Review medical records and enroll patients in Care Management.
Initiate a Care Management Plan of Care, if eligible.
Capture all diagnoses with highest specificity and address gaps.
Complete AWVs and cognitive impairment screenings.
Complete Social Determinants of Health assessments and/or screenings.
Complete ACPs to be reviewed with the patient by the provider.
Evaluate for home health, hospice, palliative, or RPM/DMEs as appropriate.
Communicate and coordinate care with the care team and patients.
