Job summary

Your Health Organization is seeking a Registered Nurse to serve patients throughout Athens and surrounding areas as a Nurse Case Manager. The nurse will visit patients in homes and facilities, coordinate with providers, and initiate telemedicine visits.

This full-time role requires travel, reliable transportation, and a valid driver's license. The team offers competitive compensation, benefits, vehicle allowance, and a supportive care environment.

Qualifications

• Must be a licensed nurse; RN preferred; license in good standing.
• Minimum of three (3) years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to manage and demonstrate effective leadership skills.
• Good interpersonal and communication skills under all conditions.
• Ability to foster a cooperative work environment.
• Team player with ability to manage multiple responsibilities and sound judgment.
• Must be able to work flexible hours and travel between offices/facilities; licensed driver with insured vehicle.

Responsibilities

• Facilitate receiving medical records from patient’s providers and specialists.
• Review medical records.
• Complete consents with patients.
• Enroll patients in Care Management if eligible.
• Initiate a Care Management Plan of Care if eligible.
• Capture diagnoses with precision and ensure acceptance.
• Complete AWVs for provider review.
• Complete cognitive impairment screenings.
• Complete SDoH assessments and/or screenings.
• Complete ACPs for provider review.
• Evaluate for home health, hospice, palliative, or consults with Your Health Specialty Division.
• Evaluate for RPM devices or tools to improve quality of life.
• Communicate and coordinate care.
• Reconcile prescriptions and OTCs, vitamins, supplements, and other treatments.
• Evaluate for adaptive equipment and DME.
• Evaluate for safe environments.
• Assess acute conditions or follow-up from previous visits.
• Document Care Management activities and keep care plans updated.
• On call may be required
